WeRHouston Oral Health Awareness Scholarship Contest

By submitting an entry for the WeRHouston Oral Health Awareness Scholarship Contest, I confirm that I am the parent or legal guardian of the student named in the entry form and that I have the legal authority to provide this consent.

I give permission for my child to participate in the WeRHouston Oral Health Awareness Scholarship Contest, sponsored and administered by WeRHouston. I understand that my child will submit an original essay and/or video related to oral health awareness and that the essay and/or video will be reviewed for contest judging and winner selection.

I understand that the information submitted through the entry form may be used to administer the contest, verify eligibility, contact parents or guardians, review essay and/or video submissions, notify finalists or winners, and award prizes. This information may include the student’s liking, name, grade, school name, student ID number if required, essay and/or video submission, uploaded essay and/or video file, and parent or guardian contact information.

I confirm that the submitted essay and/or video is my child’s original work. I understand that entries may be disqualified if they are incomplete, inaccurate, inappropriate, plagiarized,, or otherwise violate the Official Rules.

If my child is selected as a finalist or winner, I give WeRHouston permission to announce my child’s liking, first name, last initial, grade, school name, essay and/or video title, and award level in connection with the contest. I understand that WeRHouston may request additional written permission before using my child’s photo, video, full name, interview, image, voice, or likeness for promotional purposes.

I understand that participation in the contest does not require a purchase, payment, or agreement to receive other communications.

I release WeRHouston, its employees, representatives, partners, affiliates, agents, sponsors, donors, vendors, and assigns from claims arising out of my child’s voluntary participation in the contest, except where prohibited by law.

I understand that the contest is subject to the Official Rules and that the decisions of the contest judges are final. This consent is governed by the laws of the State of Texas.
